The Multiple Sclerosis Specialist Nurse aims to provide continuity of care by offering assessment, information, ongoing support and management of medication.
The nurse aims to:
Offer assessment, information and ongoing support to people affected by Multiple Sclerosis including their carers and family
Be a continuing point of contact by offering a telephone/email help and advice line, clinic appointments at various settings as well as home visits for those who are unable to get into the clinic
Be a reliable source of information for other clinicians within CSH Surrey.
Once a referral has been made, the Multiple Sclerosis Specialist Nurse (MSSN) will send out a registration form to be completed and returned. A telephone call, clinic appointment or home visit will then be arranged.
With your appointment letter, you will be sent a questionnaire for you to consider what you would like to discuss at your appointment, along with a reminder to bring your medication.
For your appointment, which will be between 30 minutes and an hour, you are welcome to bring a friend or family member with you for support. Remember: this is your appointment to meet your needs.
Once you have met the nurse, follow-up plans will be made with you to best meet your current needs.
The Multiple Sclerosis Specialist Nurse is available by phone or email to contact in between visits if there is anything that you wish to discuss or are concerned about.
