Chair sought for Board of Epsom Health and Care Alliance

Friday 10 June 2016. Expressions of interest are being sought for the role of independent Chair to lead the Board of the Epsom Health and Care Alliance.

Epsom Health and Care​ is a pioneering new partnership and service designed to make sure that older people in the area get the very best of seamless, coordinated care – whatever their health and care needs​. 

CSH Surrey is a partner in this new, integrated service, the other partners being Epsom and St Helier University Hospitals NHS Trust, Surrey County Council (SCC) and GP Health Partners Ltd (covering all 20 Epsom and Leatherhead practices). We are one of the first groups of providers in the country to come together in this way to develop such integrated care between GPs, community, acute and social care​​.

Together we have developed and are delivering a health and care service for the over 65s in the Epsom area who receive seamless and coordinated support and care delivered by one integrated team of health and social care professionals. The aim is to enable people receive the right health and social care support so they can remain safely and independently at home and are less likely to have to be admitted to hospital.

The Epsom Health and Care service currently includes social care support, access to urgent assessments and diagnostic testing at CADU (Community Assessment and Diagnostic Unit) based at Epsom Hospital, and home-based nursing care led by community matrons. In coming months we will be extending the service to also include a supported discharge service (to enable people to be discharged and return home sooner, supported by nursing and therapy input) and a rapid response nursing service (that provides urgent assessment and care at home during a health crisis)

As health and care commissioners and providers we have been working together over the last eighteen months to develop our plans to improve the care for the 188,000 residents around Epsom. We are now ready to make our first and most significant step towards making this an integrated reality. 

Our Consortium Agreement formally sets out how we will work together and will be overseen through the establishment of Epsom Health and Care Alliance Board. The Alliance Board will have representation from the four partner organisations, Lay Partner representation and representation from other key provider partners. The Independent Chair will facilitate the Board to make open, transparent decisions in a collaborative way, always using the guiding principles the partner organisations have agreed in relation to outcomes for the people of Epsom.​

Epsom Health and Care Alliance is looking to appoint a Chair for an initial period of 12 months, starting as soon as possible.  You can view  the role description for the Chair. The expected time commitment will be one and a half days a month, including attendance at all Board meetings. The remuneration for the post will be £2,500 per annum.
