What does our service do?
Our Single Point of Access (SPA) coordinates all urgent and non-urgent referrals for community nursing, intermediate care, therapy services and our medical locality hubs.
How does our service work?
The SPA enables patients receiving care from our community health services to call one number for advice or care in between planned visits.
It also enables healthcare professionals to call one number for advice on referrals, prioritising clinical need and matching a patient’s needs to the right service, avoiding the need to navigate complex systems and multiple points of contact.
The SPA operates between 8am and 6pm Monday to Friday
Who can be referred?
The SPA is for referrals into the following of our services:

Podiatry (Adult and Children)

Who can refer?
The Single Point of Access welcomes referrals from:

GPs and practice nurses
-
GP out-of-hours service
-
A&E and other hospital staff
-
Community health and social care services
-
Ambulance crews
-
Nursing and residential care homes
-
Patients and carers known to our services.
