Rapid Response Team
If you require urgent nursing care at home, your GP or other health or social care professional can refer you to our Rapid Response Service. The nurses respond within two hours of a referral and provide care for up to 72 hours. This nursing intervention can enable a patient to recover from an acute episode of illness, thus avoiding the need for a hospital or A&E admission.
The Rapid Response Nurses work as part of larger community teams that together provide holistic assessment, planning and care for people with a range of health and social care needs. The nurse will complete a full holistic nursing assessment and ensure the patient is triaged to the correct pathway, eg Social Services and District Nursing. The nurses will liaise with other services wherever necessary to ensure the patient is maintained safely in a home setting.
In a survey of 120 patients using the Rapid Response Service, 98% said they felt they were treated with dignity.

Examples of conditions that can be managed at home are:
- Urinary tract infections (UTIs)
- Exacerbation of long-term conditions such as Chronic Obstructive Pulmonary Disorder (COPD)
- Chest infections
- Falls (no injury or with minor injury)
- Sudden reduction in mobility
- Failure to cope
- End of life care.
