Services for Adults

We provide the community nursing and therapy services for adults in north west Surrey. 

To make a referral to north west Surrey (NWS) Adult Services, please use our Single Point of Access (SPA), which operates between 8am and 6pm Monday to Friday. 

Services that require referrals via the SPA pathway are:

  • Community Nursing (see separate referral emails below for weekend referrals)
  • Continence
  • Community Rehabilitation Team
  • Diabetes Nurses
  • Dietetics
  • Locality Hubs (Bedser in Woking, Ashford Hub, Walton Hub)
  • Heart Failure
  • Medicine Support
  • Neighbourhood MDT
  • Phlebotomy
  • Podiatry (adult and children)
  • Respiratory service
  • Speech and Language Therapy (SLT)
  • Tissue Viability Team
  • Wound Clinics

The following services do not take referrals via the SPA pathway:

Weekend referrals to Community Nursing

  • To make a referral to services in the Surrey Downs area, please contact the Referral Management Centre (RMC), which is open Monday to Friday 8.00am - 6.00pm (excluding bank holidays):
